Order in 4 easy steps:

Step 1. Fill out the order form below. (If you want to mail in your order form with a check/money order then please do not use this form.  Instead, please print and mail in the order form.)

The cost of tests includes Shipping/Handling and can be paid online using either a Visa or Mastercard or Discover.

Step 2. Preview your order on the next page.  Make sure everything is correct.  You may also add other items to your cart at any time, if you so desire.

Step 3. Enter the shipping and billing address.  The tests will be shipped to the shipping address, and the billing address should be the address where you receive your credit card bill.

Step 4. Enter the credit card information on the secure payment page.

If you're ordering for a group, the group discount prices will be calculated automatically.  If you need extra directions for administration please remember to enter that information.

GROUP DISCOUNTS: Tests for groups must be ordered a minimum of 4 weeks in advance by one person.  All test materials and test results will be mailed to one person. The discount is automatically calculated.

10-29 tests = Discount $2 per test
30+ tests = Discount $4 per test

EXTRA TEACHER'S GUIDES: Grades 1, 2, and 3 each have separate working times and, therefore, each grade must be tested separately.  However, grades 4-12 all have the same working times and can, if your space permits, be tested together.  Therefore, we will send you one(1) "Examiner's Manual " per each of these student groups. If you need additional Examiner's Manuals, please list the number of additional directions below. These are only for GROUPS of 10 or more students, testing simultaneously at the same location on the same day; please do not order them otherwise.  Thank you!
